Scope

The contractor shall provide Ground Ambulance Services at Fort Polk, LA in accordance with the Performance Work Statement included in the SF1449. All offerors shall provide the following items: Please complete the SF1449: Blocks 17 (to include the CAGE code and the Unique Entity ID Number) and 30 A - C. Please provide a statement that the offeror will hold its price firm for at least 90 days from the due date for receipt of proposals, subject to any changes that may occur as a result of discussions (if applicable). Please complete the provisions, as listed in the solicitation. Alternately, complete all provisionsin System for Award Management (SAM) and provide confirmation of completion and accuracywith offer.
